What you’ll be doing
- Leading intake & assessments — processing My Aged Care referrals, conducting face‑to‑face assessments, and ensuring every client receives a respectful, person‑centred entry experience.
- Building strong partnerships with hospitals, pharmacies, service providers, and community organisations to strengthen access and collaboration.
- Supporting client independence by identifying needs, setting goals, and ensuring timely commencement of services.
- Maintaining high‑quality documentation that meets Aged Care Quality Standards and Support at Home compliance.
- Contributing to continuous improvement through data insights, reassessments, and collaboration with Managers and Clinical teams.
- A passion for person‑centred care and culturally sensitive practice.
- Strong communication and customer service skills.
- Experience in aged care, community services, or health.
- Confidence using CRM systems and the My Aged Care portal.
- A warm, professional approach that puts clients and families at ease.
